Enrollment Application

Upon enrollment all applicants must:

• Possess a valid driver’s license in your state of residence
• Qualify to obtain a valid Commercial Learner’s Permit
• Pass a DOT drug screen
• Obtain a valid DOT physical (Recommended to have this done prior to enrollment, but not required)
• Be approved by an admissions representative, and complete an enrollment agreement
• Be 18 years of age or older
